			<content:encoded><![CDATA[<p>As we head down the road of revising our health care system, there are pros and cons to practically every angle of the legislation. Many people have doubts as to whether anything can really work as evidenced by the snapshot of opinions (over 1100 at the time of this posting) on a <a href="http://www.medpagetoday.com/Surveys/">MedPage survey.  </p>
<p><a href="http://content.nejm.org/cgi/content/full/NEJMp0910064?resourcetype=HWCIT">A very recent article regarding the ability to do a quicker, iterative review and change of negative effects of the legislation, England Journal of Medicine, wherein we are also reminded of the statistically poor mortality outcomes we have in the United States relative to our expenditures. Othere changes thought by many but not covered in the legislation include tort reform, addressing access issues, increased insurance transparency via separation from employment, and piloting of changes before nationalizing them. </p>
<p>But then again, some of the issues are reflective of our life style choices which can be hard to legislate. Perhaps that is where education steps in? At the end of the day I still remain in favor of education over legislation but am hoping for the best.</p>

			<content:encoded><![CDATA[<p>This sounds like good news and it is for those that qualify. There is a heatlh care tax credit (HCTC) of 65% for someone purchasing their own health care insurance coverage if you qualify. It would be tremendous if this coverage was for all individual purchasers - this is the hope for the future. This is not tax advice in any way but rather, information in case it is helpful to you.</p>
<p>Here is a link to the Internal Revenue Service HCTC program guide entitled <a href="http://www.irs.gov/pub/irs-utl/06_04_program_kit.pdf">Health Care Tax Credit Program Kit.</p>
<p>In general, <strong>you may claim this credit if:</strong><br />
- You are receiving certain Trade Adjustment Assistance (TAA) benefits,<br />
- You are receiving benefits under the Alternative Trade Adjustment Assistance<br />
(ATAA) program, or<br />
- You are receiving benefits from the Pension Benefit Guaranty Corporation<br />
(PBGC) and you are at least 55 years oldIn general, HCTC</p>
<p><strong>You are not eligible</strong> for the HCTC if any of the following applies to you:<br />
- You are enrolled in a health plan maintained by your or your spouse’s current<br />
or former employer that pays at least 50% of the cost of coverage<br />
- You are entitled to Medicare Part A or enrolled in Medicare Part B<br />
- You are enrolled in the Federal Employees Health Benefits Program (FEHBP),<br />
Medicaid, or State Children’s Health Insurance Program (SCHIP)<br />
- You are entitled to health coverage through the U.S. military health system<br />
(TRICARE/CHAMPUS)<br />
- You are enrolled in a qualified health plan (described on page 7 of the program guide, other than<br />

			<content:encoded><![CDATA[<p>In January, 2010, Bellevue College will offer a new Healthcare IT Curriculum for experienced IT workers to learn how to transition into the health care sector. Basically, Healthcare Information Technology is a noble and growing industry - an estimated 30,000 - 50,000 new jobs will be required in the near future. Why? Because not only did George W. Bush call for America&#8217;s health records to be electronically managed by 2014, the Barack Obama Administrations advanced legislation in the ARRA (Stimulus Recovery Act of 2009) that financially supports the preparation and adoption of electronic health records. The clinical and business value that electronic health records can offer our country is tremendous and the opportunity and necessity to do it is now. </p>
<p>Starting in January, 2010, Bellevue College will offer experienced IT professionals the education they need to transition their career into the healthcare arena. In order to make a successful move, through the Certificate Program, the IT professionals will become educated about:</p>
<p>·  The language and sub-culture of healthcare (Medical Terminology)<br />
·  The background of the healthcare system and its technology adoption (Healthcare Fundamentals)<br />
·  The nuts and bolts of data management in healthcare (Healthcare IT Data Standards)<br />

<p>The virus directly infects the lung and can cause severe respiratory failure which requires intensive care. The Southern Hemisphere has already experienced a higher demand on their Intensive Care Units (ICUs) because of this fact. Unlike the seasonal flu virus, H1N1 affects otherwise seemingly healthy younger people. The good news is that the majority of cases have not been severe so far according to the World Health Organization which is the source for this information.</p>

<p>June 27th was the National HIV Testing Day. The date came and went with little publicity as far as I could tell but I received an FDA notice on July 13th stating as much. The message stated that someone is newly infected with the virus every 9 1/2 minutes. Ouch!</p>
<p>The CDC has a website where you can enter your zip code to find a local testing center <a href="http://www.hivtest.org/"> www.HIVtest.org. This website also has a lot of excellent information on it as well, like how long after a possible exposure should you wait before getting tested, etc. Another easy way to find a testing center using your phone is to send a text message to 566948 (&#8221;KNOW IT&#8221;) and you will text message back with a center near you. <strong>I put in my own zip code and found 5 free testing sites with a 25 mile radius of me - it&#8217;s that easy!</strong></p>
<p><strong>Why get tested? </strong> If you know, you can make more informed decisions and those who catch their status in early stages of the disease can improve and extend their lives as opposed to those who do not get tested and then get diagnosed at a later state.</p>

			<content:encoded><![CDATA[<p>Most Americans wants some sort of health care reformation - many of us just don&#8217;t want it to be a complete and total change immediately. People take time to absorb change and we&#8217;ve already had lots of government change over the last eight months. Regarding heatlh care, things are heating up in this country and it is interesting that we see an article of the AFL-CIO sending in members to ensure a balance at the town hall meetings.  </a><a href="http://online.wsj.com/article/SB10001424052970204908604574334623330098540.html">http://online.wsj.com/article/SB10001424052970204908604574334623330098540.html</p>
<p>It is unfortuanate that a lot of mud is being slung in every direction and it is hard to understand the truth. My dad used to say there were three sides to every argument: yours, mine and the truth. Okay so whatever; how do we peacefully move forward from here?</p>
<p>Let me pose an idea as a way out for everyone to save face and help the nation keep moving forward in a progressive manner towards a better health care delivery system. Let’s move our dialogue and focus on Evidence-based Policy so that we can cautiously and confidently transform our health care policies, delivery system, legal ramifications, etc. What does that mean and how does it look?</p>
<p>Most big changes, projects, or whatever, use a method of testing the integrity of a system by piloting the changes in a smaller environment rather than a sweeping change to the larger entity overall. This makes a lot of sense as one can evaluate what is working and what is not. With the health care legislation and transformation efforts, the United States has a perfect test site - the State of Massachusetts. <a href="http://www.nytimes.com/2009/07/30/us/30immigrant.html">http://www.nytimes.com/2009/07/30/us/30immigrant.html</p>
<p>Massachusetts mandated that everyone obtain health care insurance coverage in 2006 and have about a 98% compliance rate although this rate is falling because of the economy. Recently, the state has requested that they be free from Medicare &#038; Medicaid guidelines in order to move towards a per person / per month reimbursement model. Sound like an HMO - yes but the reason it could be different than the HMO&#8217;s of the 90&#8217;s is that there could be no &#8216;cherry picking&#8217; or denial of individual coverage. Cherry picking by insurance companies was associated to the failed governments attempt at HMO&#8217;s in the 90&#8217;s (otherwise known as Medicare Part C). </p>
<p>Anyway, it seems we would have a greater chance of success with national health care reform if first focus on a pilot state. If the federal government worked with Massachusetts focusing on their cost over runs, examining the benefits of individual reform components that would greatly improve our health care system. These include:</p>
<p>1. Per person / per month reimbursement model to reduce wasteful paperwork and provide more physician to patient time (currently less than ten minutes per visit).</p>
<p>2. Tort reform which has helped to reduce insurance cost in the states of Texas and Ohio.</p>
<p>3. Transparency of information: total costs and patient out of pocket fees before expenses are incurred; provider report cards (performance evaluations) in consumer friendly formats; easy and complete access to evidence-based treatment protocols, etc.</p>
<p>4. Create equality for obtaining health insurance regardless of how it is bought. This could look like State Tax incentives for obtaining health care insurance and also taxing employer based insurance (over the equivalent tax credited amount). </p>
<p>If we could enact changes and monitor with laser sharp analysis of outcomes, I believe the American public that has questions about cost and methods of the enormous legislation proposed would feel much better. We would have a better idea of the change and the true consequence. Because as of now, the Congressional Budget Office (CBO) calculates a trillion dollars addition to our looming debt associated with the bills on the table at this point. <a href="http://www.washingtonpost.com/wp-dyn/content/article/2009/07/16/AR2009071602242.html"> http://www.washingtonpost.com/wp-dyn/content/article/2009/07/16/AR2009071602242.html </p>
<p>And that calculation does not include any consumer behavior change based upon the policy changes which seems like a glaring flaw. After all, the stock market is so fascinating because of the psychological response to news, etc. that causes massive fluctuations. Investors are consumers of health care too; consumers will have psychological responses to any health care reform put before them.</p>

Choice regarding health care services is a economic priviledge for many of us that I hope we can maintan. Wednesday&#8217;s <em>Wall Street Journal</em> had a good article, written by economist George Newman, that quickly went through the logical considerations one would make regarding arguments for health care reform. It is a helpful piece if you are still trying to make sense of the optimistic assertions made by those who believe that everyone is in favor of what some in the government are promoting. There remains a lot of opposition to the reform package being presented.<br />
